Each person attending training must complete this registration form. There are two (2) trainees included with the original program fee paid by your institution. The cost for any additional trainees is $750/person.

Cost for meals for the three-day training is typically $85/college attendee. Please note that all attendees are required to pay the cost for meals. This cost will be billed after the training has been completed. Any sponsorship money raised for the training will be used to offset these costs and will be evenly divided amongst the three Texas training events.

Registration is limited to 70 trainees per session. Registration is on a first come, first served basis.

Please note that some of the registration information will be used to compile a contact sheet that will be utilized to communicate during the actual peer audits. We are taking advantage of this opportunity to collect the information (i.e., cell phone number) rather than request it at another time. Thank you for your assistance in this effort.
